American Cancer Society  Relay For Life West Bend - 2006 A Community Fundraising Event to Fight Cancer 2006 WEST BEND RELAY FOR LIFE TEAM CAPTAIN INFORMATION

Gather 8-15 co-workers, family members, friends, neighbors, church members, and anyone else interested in fighting cancer. We encourage all ages and groups to participate in the American Cancer Society Relay For Life. Remember the people you are gathering should be willing to raise money and donate some of their time to participate in the event.

Ask a local business, organization, your own place of employment, or your team members to pay the commitment fee of $150. Early registration Fees are due June 2nd. However, entries will be accepted up to the date of the event. Please make checks out to the American Cancer Society. Also bring T-shirt order form and team logo and fee along with each team members’ name, address, phone number and T-shirt size. This is very important so each team member is counted and T-shirts are ordered correctly.

Each team member is encouraged to raise $100 and teams are encouraged to raise $1500. There are several strategies for fundraising included in this packet, or if you are in need of more fundraising information contact  Larry Koziol at 262-338-1996 or Chris Weston at 262 338-1996.

There will be a "Kick Off Meeting" will be held on Wednesday April 12th at the West Bend Clinic in the P.T. room on the lower level. Please use the main entrance. All team captains and others that are interested are welcome to attend.

Team Captain Meetings
Informational Meeting February 23rd, 2006 at 6:30 p.m. at the West Bend Clinic.
Meeting to turn in final registrations and T-shirt order May 25th at 6:30 p.m. at the West Bend Clinic.
Final Questions Meeting July 27th 6:30 at p.m. at the West Bend Clinic.

Participate in the entire American Cancer Society Relay for Life activities, including campsite decorating, food service, games, live entertainment, and much, much more! An entertainment schedule and several flyers will be sent with updates on the activities at the Relay For Life.

A member from each team should be on the track at all times during the event. We do not compete for most laps or most amount of time on the track. Incentives are given out for those individuals raising over $250 and those teams raising over $1000 will receive a American Cancer Society Relay for Life plaque to display.

The West Bend American Cancer Society Relay for Life is held at Washington County Fair Park. Alcohol and smoking are strictly prohibited. School rules also prohibit pets, roller blades, and skateboards on the walking area. Strollers and wheelchairs are allowed for this event.

Teams are strongly encouraged to turn in their funds raised at the Bank Day  The Bank Days will be held to allow team captains can take team money.  Bank day is August 3rd, 2006  from 9:00 a.m. to 7:00 p.m.at Trinity Lutheran Church. (use west entrance off ally)

To be announced  final date to turn in team money to be credited to the team totals for the Relay. Money turned in after that time will be accepted, but not be included in team total information available at the American Cancer Society Relay for Life on August 4th and 5th.

Please make all checks payable to the American Cancer Society.

An incentive for those individuals raising over $1000 is the Star Supporter Award. This award is a bright colored T-shirt that can be worn at the event to promote the fact that the wearer of the shirt raised $1000 for the fight against cancer. In order to receive this special incentive all money must be turned in by (time to be announced).
