Relay For Life West Bend - 2007

Team captains received packets of information at the Team Captains Meetings or from the ACS Office. Some of the following material were included in the packets, and some were not. These documents can be downloaded in Microsoft Word format from the web page by clicking on the links.
Pledge Form - Form used by team members to record pledges from friends, relatives and others
Extreme Prize - To use with fundraising to register for opportunities to win extreme prizes (NOT AVAILABLE UNTIL 2006 PRIZES ARE SECURED)
Team Member Walking Schedule - To be used to schedule team members for a time to be walking on the track
Event Rules - These are the rules that participants are asked to follow at the event
Team Sales Certificate - To be completed by any team planning to sell items at the Relay
Chaperone Form - To be completed to designate chaperones for student teams
Student Permission - To be completed by students and signed by parent or guardian for youth participants.
Luminaria - For ordering luminaria for the Relay
Silent Auction Donation Form - Complete this form and attach to donated items
Relay For Life Cash Receipt - In Word format, use for cash donations.
